Mid Staffs scandal: NHS chief must not be a scapegoat, says PM
Mr Cameron insisted that Sir David, right, has  explained himself well

Ultimately, Sir David Nicholson, the chief executive is responsible for what has happened and why it was not exposed sooner, notwithstanding that he was not CEO in the early days of the scandal. However, those individuals and those responsible for those individuals who were part of the abuse and the subsequent concealment of the truth, must be subject to disciplinary actions and where necessary criminal charges.
