I am sure that from this latest case, the facts will be examined (again) and the subsequent enquiry will report (again) that many factors were missed and that lessons will be learned (again). An appalling catalogue of failures in the Health Services, the Social services, the police and other bodies have plagued our society for far too long. It may well be that "the systems" contribute to the failures, but the systems are designed and operated by real people who frequently hide behind the excuse of systems failures for their own incompetence.
